At all times, including while being stored, prepared, displayed, served, received, or transported, food not protected from potential contamination by all agents, including dust, insects, rodents, toxic materials, cross-contamination, damaged or spoiled products, distressed merchandise, recalled products, unclean equipment and utensils, unnecessary handling, coughs and sneezes, flooding, draining, and overhead leakage or overhead drippage from condensation.

The internal temperature of time/temperature controlled for safety (TCS) food not maintained at 41°F (05°C) or below or 135°F (57°C) or above at all times, except as otherwise provided in this article or unless a packaged product is so labeled as exempted and approved by the appropriate state or federal health authority.

Ventilation hoods. Exhaust ventilation hood systems in food preparation and warewashing areas, including components such as hoods / fans / guards / ducting, not designed to prevent grease or condensation from dripping or draining into food or onto food / equipment / utensils / linens / single-service and single-use articles.

Hand-washing sink faucets. Hand-washing sink not equipped to provide water at a temperature of at least 100°F (38°C) through a mixing valve or combination faucet. Hand-washing sinks in facilities providing custodial care for preschool age children that are used only by the children may provide only cold running water.

Hand-washing sink installation. Hand-washing sink used for purpose other than hand-washing as specified in section 20-21.08(b) of this code.

Openings. Openings to the outside not effectively protected against the entrance of rodents.

Floors. Floors and floor coverings of all food preparation, food storage, utensil-washing areas, walk-in refrigerating units, dressing rooms, locker rooms, and toilet rooms shall be constructed of smooth durable materials such as sealed concrete, terrazzo, ceramic tile, vinyl, plastic, or tight wood impregnated with vinyl or plastic.

Special ventilation. Filters not cleaned at sufficient frequency to prevent accumulation.

Locker areas. Enough lockers / other suitable facilities not provided / used for the orderly storage of employees' clothing and other possessions. Lockers or other suitable facilities may be located only in the designated dressing rooms or in food storage rooms or areas containing only completely packaged food or packaged single-service articles.
